(Fill out and turn in to the FCCS office.  An appointment will be scheduled and upon acceptance payment of  registration will be required to hold place.)

  5. Character References (2 references must be filled out for 1st - 3rd ; only 1 for 4th - 12th)
  6. Discipline Reference (1 reference needed for 4th - 12th; none for 1st - 3rd)

Returning Students:

Re-registration packets were mailed home in January.  If you need another, please contact the school office at (904) 777-3040.
